Common Threads: Diabetes Self-Management Program

The Diabetes Self-Management Program – Common Threads, originally developed at Stanford University, is for people with diabetes. Sessions are held 2½ hours once a week for six weeks.

Participants learn about making a healthy eating plan, preventing low blood sugar, balancing eating, physical activity and medications to prevent problems, as well as managing diabetes-specific problems.

Participants receive a FREE companion book and a relaxation CD!

Philadelphia Corporation for Aging offers the Diabetes Self-Management Program, an evidence-based program originally developed at Stanford University, under the Pennsylvania Department of Aging’s license with the Self-Management Resource Center.
